By submitting this application, you are indicating your interest in receiving a free Ojo video phone and would like to begin using Snap!VRS. Please note this application is for new customers only. How to Apply:
To apply for a free Ojo video phone, please follow the instructions below:
1) Review the eligibility requirements
You must meet the following requirements before submitting an application:
- You are a new Snap!VRS customer
- You are a U.S. citizen
- You are Deaf, Hard-of-Hearing, or Speech Impaired
- You use sign language to communicate
- You have a valid telephone number (or we will provide you with one)
- You have access to high-speed Internet (e.g. Cable, DSL, or FiOS). Snap!VRS recommends a minimum connection speed of 256 Kbps
- You have a valid address for receiving 911 emergency services and package deliveries (i.e. P.O. boxes are not accepted)
- You are at least 18 years old or have the approval of a parent or guardian
2) Complete and submit the Application for a Free Ojo Video Phone
This form includes basic contact information such as your mailing address and preferred method of contact, as well as other information about you. NOTE: If you are under 18 years old, you also need to complete and submit the Guardian Permission Form. If you are under 13 years old, we cannot accept an online application in accordance to the COPPA Act. You must have your parent or guardian download the PDF application and mail or fax it to us.
